This is a follow-up to the letter you received regarding our participation in the Medicare Shared Savings Program (MSSP) through an Accountable Care Organization (ACO). An ACO is group of providers, clinics, hospitals and other healthcare professionals dedicated to providing coordinated, high quality and cost effective healthcare.
Our participation in the program aims to improve the quality of care you receive while potentially lowering healthcare costs by increasing care coordination.
Beneficiaries should have a clear understanding of the program and its potential impact on their care.
Questions may arise about:
- How your care is coordinated: How care is coordinated to ensure seamless and effective care
- Medicare data sharing: How Medicare shares health information with healthcare providers within the ACO to help coordinate care.
- Your right to choose providers: The freedom to choose any Medicare provider, inside or outside of the ACO, remains.
Important reminders:
- Medicare benefits will not change due to MSSP participation. You do not need to do anything. You can continue to see the same provider.
- You have the option to decline sharing claims data with the ACO. Call 1-800-MEDICARE (1-800-633-4227) for any questions or to decline.
- Additional information about Medicare and privacy rights is available at Medicare.gov by searching "privacy".
Our Goal:
The goal is to prioritize preventive care, such as the Annual Wellness Visit, preventative screenings, and immunizations to maintain health and quality of life.
